Enter the Fog Mutation

This mutation was inspired by foggy vision by AlwaysToast, its similar but with a few tweaks that i wanted to add to play with my friends:
- set on campaign, but bodyshot damage is a bit higher than default realism and lower than default coop
- the fog is further away from the player compared to foggy vision
- common infected attacks deal up to 30 damage (on expert)
- slightly reduced ammo
- survivors can crawl when downed but very slowly, just enough in case you get stuck somewhere your friends cant reach
- double special infected can spawn and will spawn faster
- other minor tweaks
