If you go into this game expecting a realistic damage model shooter, you will be disappointed.

The survival mode is also in a middle of a massive snow storm, it's really cold, and you have to build up cold resistance by making/looting clothing.
PVP and PVE modes of survival are available, and the mode gives you gear caches (crates that when opened in your inventory give you random items) as rewards, which in survival each cache will contain between 2 and 4 pieces of gear.
Rewards in this mode, come in two types: Rewards based on score (up to 4 caches depending on how well you do) and rewards based on what you loot in the survival modes Darkzone and extract with, in other words, you need to find these and successfully finish the survival mode (upto 6 sealed caches).
In conclusion then, to answer your question simply:
No, the season pass isn't required, but you'll be missing out on some of the two biggest activities added to the game via the DLC's you'd get with it.
As for survival, this is a 24 man activity. So you have to matchmake to get in. This dumps you in a modfied version of the main world map, you start off with next to nothing and have to gather materials, craft gear and clothing and make your way to the darkzone to extract.
Difficulty is added here with a few special mechanics for the mode. Hunger, Thirst and Infection. Hunger and Thirst will affect your ability to do things, and must be sated/quenched with food and drink. The infection, on the other hand, is basically an hour timer. When that reaches zero, you die. The infection timer can be halted by using medicine.
Underground, is basically, procedually generated missions. It's good fun, but the procedual generation works much like Diablos. There is a bucket full of prebuilt area's that it connects together randomally. Which means, eventually, you'll have seen all the pieces.
However, you can mix things up a bit in underground, with the phases and directives. The more phases you add, the more back to back underground missions you have to do for a successful run, and Directives add modifiers to the experience to make it more difficult.
In both cases, rewards are increased.
