How to unlock hard and insane mode?

Guys how to unlock and insane mode? i beated normal and easy modes.

Resolved: "Only for the Hardcore" Achievement not Unlocking

Update: Issue has been resolved. If you had previously unlocked Insane, completing any level on Hard should unlock the achievement. Strangely, the achievement for unlocking the Insane difficulty didn't trigger when I 1CCed hard. But I could go on to the Insane difficulty, and even get the achievement "Assault Suits Saladin" (Clear a stage on Insane Mode without taking damage and with at least 240 seconds left on the timer). Did anyone manage to get the achievement upon 1CCing har...
