Story related. Immediately after meeting the Old Woman, you will have to lure and trap Kathy in a nearby room. Once you do, the achievement will unlock after starting the next section of the game.

Story related. This will unlock after loading the hard drive on to your computer in the office.

Interrupted the performance for ghost.

Story related. After finding the Amber Glasses and going through the secret door on stage, you'll find yourself in the middle of an opera. Approach the stage to unlock the achievement.

Are they really exist?

Story related. This will unlock after you finish the obnoxious PT-like back and forth section in your office.

The Clue
Truth is waiting for you.

This is earned in chapter 5. After escaping the Lady in Red for the first time, you'll drop into an apartment with a bunch of notes describing a sick child. Exit the room and eventually a large Ferris wheel will appear. Locate the area where you can hop over a railing on to a rollercoaster-like track, but don't go anywhere just yet. Instead, look off the side to the right and you'll notice that you can hop off the track and land on top of one of the carriages. Ride this up until you spot the marking in the picture below.










From here, you'll notice a separate piece of track you can hop off on to (you'll need to do this to reach the sheep and train dolls). Do so, and follow the track a few feet upwards. You'll notice that you can hop over the railing on the floor beneath the train marking.










As soon as you hop over, there will be a table straight ahead with a note on it. Read it and put it back down to unlock the achievement.

Triathlon
Thanks little girl.

Story related. This will unlock after running the Lady in Red over with the car enough times. This fight can be really annoying and easy to die on. My best advice is to always have your finger on either the W or the S key, as this will make the car move and turn faster. After bumping into her, spin the car in circles and try to locate her again as fast as possible.

Semi-story related. After finishing the game, make sure you watch the credits through to the end. Don't worry, they're very short. Afterwards, a bonus scene will play. Watching this cutscene is what triggers the achievement.
