Disclosure
I am proposing this guide for all-around users to unlock 100% of achievements in the fastest way. I know that you can reach 100% even faster in some games for a professional gamer. However, I am aiming for guides that will also benefit users with no experience with the game. So normally, there are no suggestions like doing the first run in the hardest difficulty.

SPOILER ALERT is in order as some details about the story of the game will be revealed here.
First Run
In this run, you are going to enjoy the story in the minimum difficulty, “Assisted”, and get 29 out of 46 achievements along the way. Time-wise, this would be your longest run and you should investigate every corner of the map for side missions, treasures, etc.

I am breaking the achievements into two groups:
1. “Continuous Achievements” are the achievements that you must do and trace in different parts/chapters of the game.
2. “Standalone Achievements” are the achievements that you must do in a specific location of the game.

Some notes for this run:
1. Remember to save a lot and use all your slots so you can backtrack if something is missed.
2. Some achievements are rather hard to do on the first try. So, make sure that you save before approaching the event to have it covered. I have put the word "MISSABLE" in the achievements list for you to spot them more easily.
3. Don’t get too easy with your spending. Try to gather as many as resources you can to put together some good money. You will need this to buy some stuff from the merchant for your next playthrough.
4. Important: Keep your save file at the latest Merchant just before the endgame.
Standalone Achievements
Knife Basics
Parry an enemy with a knife.
Chapters Involved: Any
In-Game Reward: Nothing
Just wait for any enemy in any part of the game to do a charge attack and just use the “Parry” button when you get the prompt on top of your weapon marker on the bottom-right of the screen. The lower the difficulty, the easier it would be to do the parry as the indicator will stay longer as feasible.

Revolt Against the Revolting
Destroy a Clockwork Castellan
Chapters Involved: All chapters, as there is one per chapter
In-Game Reward: Nothing
MISSABLE!
There is a total number of 16 Clockwork Castellan in the entire game and shooting just one of them will earn you this achievement. There is a continuous achievement for shooting all of them in the entire game and that is what we are looking for in the first run. Here is your guide video to find one out of 16:

Nice One, Stranger!
Complete a request for the Merchant.
Chapters Involved: 1, 2, 3, 4, 5, 7, 9, 10, 11, 12, 14, 15
In-Game Reward: Nothing
MISSABLE!
There are a total of 19 Merchant requests in the entire main story and you need to do just one of them to get this achievement done. There is a continuous achievement for doing all of the 19 requests of Merchant in one playthrough and that is what we are looking for in the first run. In the following link, the notice pick-up, the how, the where, and the when of all 19 requests is shown:

Shield Your Eyes
Defeat 3 enemies at once with a flash grenade.
Chapters Involved: 3 (Easier to do there)
In-Game Reward: Nothing
Your easiest way to do this is in Chapter 3 in the yard where there are too many crows. Just remember to have a flash grenade equipped and have it done by simply throwing the grenade onto them. There is a guide here:

Amateur Shooter
Complete a game at the shooting range.
Chapters Involved: 3, 9, 11, 14, 15
In-Game Reward: Nothing
MISSABLE!
This is rather an easy achievement as you just need to complete ANY of the shooting range games. There is a continuous achievement for getting S rank in all of the shooting range games and that is what we are looking for in the first run. If you do that in your first run, then this achievement will get unlocked in the process as well. The video linked below will show you how to complete all of the shooting range games:

Harpoon Hurler
Defeat Del Lago.
Chapters Involved: 3
In-Game Reward: Nothing
This is part of the story, so you are going to do it eventually. Just kill the bastard and the achievement is yours.

My Preferred Piece
Upgrade a weapon.
Chapters Involved: Any
In-Game Reward: Nothing
This is rather an easy achievement as you just need to upgrade a weapon. Simply gather some money, head to the Merchant, and do an upgrade for any weapon of your preference.

Talk About Near-Death Experience!
Rescue Ashley as she’s being carried away by the enemy.
Chapters Involved: 5, Also anywhere with Ashley on your side.
In-Game Reward: Nothing
MISSABLE!
This is a very easy achievement. You just need to shoot the enemy that is carrying Ashley. I am showing the very first part of the game that you can do this... but there will be plenty of opportunities to do this later in the game. Be careful not to shoot Ashley. Here is a quick guide on how to do this one:

Grilled Big Cheese
Defeat Bitores Mendez.
Chapters Involved: 6
In-Game Reward: Nothing
This is part of the story, so you are going to do it eventually. Just kill the bastard and the achievement is yours.

Overkill
Use a canon to defeat a Zealot.
Chapters Involved: 7, 8
In-Game Reward: Nothing
MISSABLE!
Either in Chapter 7 or in Chapter 8, where you get control of big canons, you have to use that to kill a zealot. Simply aim and blow up the enemy to have the achievement in your pocket. In Chapter 7, zealots will come toward you on the path to the canon itself. In Chapter 8, you can use the canon to kill the enemies standing in front of the gate you have to blow up for story progression. Here is a guide video to showcase how it should be done:

Never Heard It Coming
Defeat a Garrador using only knives.
Chapters Involved: 7
In-Game Reward: Nothing
MISSABLE!
It is better if you try for this achievement in easier difficulties as you have to do less damage to Garrador. Just be patient and lure him in a way that you can get behind him. Use your strongest knife to do the achievement, though I used kitchen and boot knives to show the worst case. Do NOT postpone this achievement as this chapter is the most isolated and easiest time to do the achievement. Here is your guide video for this one:

Astute Appraiser
Sell a single treasure for at least 100000 ptas.
Chapters Involved: Anywhere with a Merchant, but the suggestion is Chapter 10.
In-Game Reward: Nothing
This is a very easy achievement. You just need to pick up a crown off a dead body before the fight with Verdugo and attach the jewels as in-lays onto it. In the video below, you can see where to pick the crown up and which combination will grant you the achievement. Remember to sell the treasure to the merchant to get the achievement.

A Masterpiece
Get the exclusive upgrade for a weapon.
Chapters Involved: Any
In-Game Reward: Nothing
This is rather an easy achievement as you just need to upgrade a weapon to its fullest. Simply gather some money, head to the Merchant, and do all the upgrades for any weapon of your preference. Once you do that, you get an exclusive upgrade for that weapon. Either use a ticket from the merchant or just pay for the upgrade to be done and you have your achievement unlocked.

Wave Goodbye, Right Hand
Defeat the Verdugo.
Chapters Involved: 10
In-Game Reward: Nothing
This is part of the story, so you are going to do it eventually. Just kill the bastard and the achievement is yours.
Standalone Achievements Cont.
Hope You Like Thrill Rides!
Make it through both minecart sections in the underground tunnel without taking any damage.
Chapters Involved: 11
In-Game Reward: Nothing
MISSABLE!
This one is rather tricky so remember to save before getting on the carts so you can redo it in case you miss. During the rides, you must shoot the enemies as fast as you can, otherwise they will get to you. With lower difficulty, there is a good chance they miss you and that is why you are doing this in this run. For Dr. Salvador (Chainsaw guy), you need to shoot him in the head to stun him. Do that ONLY after he gets close to you for better effect. Remember to lean in the opposite direction of the ride as soon as Luis tells you to. There is a video here as a guide:

Capacity Compliance
Reach the top of the clock tower without the lift stopping once.
Chapters Involved: 12
In-Game Reward: Nothing
MISSABLE!
This one is rather tricky so remember to save it before going to the clock tower so you can redo it in case you miss it. It is better if you try for this achievement in easier difficulties as you can take out enemies more easily, hence the first run in assisted difficulty. It is okay if a few enemies jump on the platform as well ONLY IF you take them out very quickly. Anyways, if the platform stops working, then the achievement is lost. Here is your sample guide on how to do this one:

You Talk Too Much!
Throw a grenade into Ramon Salazar’s mouth.
Chapters Involved: 12
In-Game Reward: Nothing
MISSABLE!
This one is rather tricky so remember to save it before the fight so you can redo it in case you miss it. Salazar is moving too often, and your best chance is when he is taking damage or right at the beginning. You must make sure to do some damage and do the grenade shot when he is moving slower. There is a guide here:

No Thanks, Bro!
Defeat Ramon Salazar.
Chapters Involved: 12
In-Game Reward: Nothing
This is part of the story, so you are going to do it eventually. Just kill the bastard and the achievement is yours.

Two Bugs, One Stone
Kill 2 parasites inside a Regenerador with a single bullet.
Chapters Involved: 13
In-Game Reward: Nothing
MISSABLE!
You definitely want to do this achievement in Chapter 13, where the Regeneradors are locked inside those big containers. Just use your sniper with the bioscope to locate the bugs inside them and see which two bugs on which Regenerador would do the trick. Mostly it is seen that this will require you to do the shot from one side of a Regenerador as shown in the video below:

Trick Shot
Shoot through and destroy 5 targets at the shooting range with a single shot.
Chapters Involved: 14, 15
In-Game Reward: Nothing
MISSABLE!
You may want to do this achievement in any shooting range that allows you to do a 4-A game. That is essential to the easy way of getting this achievement as you will have a Punisher handgun with the penetrating feature. The rest is easy... just get the targets lined up and shoot 5 with one bullet. Check out the video below to see how it is done:

You Used to Be a Good Guy
Defeat Jack Krauser.
Chapters Involved: 14
In-Game Reward: Nothing
This is part of the story, so you are going to do it eventually. Just kill the bastard and the achievement is yours.

You’re Small Time!
Defeat Osmund Saddler.
Chapters Involved: 16
In-Game Reward: Nothing
This is part of the story, so you are going to do it eventually. Just kill the bastard and the achievement is yours.

Smooth Escape
Escape on the water scooter without taking any damage.
Chapters Involved: 16
In-Game Reward: Nothing
MISSABLE!
This one is rather tricky so remember to save before trying to escape the island with Ashley so you can redo it in case you miss. Some very small objects do not damage the scooter, so it is fine to just slide past those. Again, easier difficulty means you have more time to escape so that is why we do it in the first run. Here is your quick guide on how to secure this achievement:

Continuous Achievements
Bandit
Obtain all treasures indicated on the village treasure map in a single playthrough.
Chapters Involved: All Chapters
In-Game Reward: Nothing
MISSABLE!
There are 39 treasures in total for the village. If you gather all the treasures in ONE playthrough, you can unlock the achievement. These achievements have nothing to do with game difficulty. However, please do note that you need to do some backtracking or go off-route to get the treasures. That is why it is better to do them in your first run. In the following guide, you can find the location of all treasures in the main game:

Burglar
Obtain all treasures indicated on the castle treasure map in a single playthrough.
Chapters Involved: All Chapters
In-Game Reward: Nothing
MISSABLE!
There are 41 treasures in total for the castle.

Raider
Obtain all treasures indicated on the island treasure map in a single playthrough.
Chapters Involved: All Chapters
In-Game Reward: Nothing
MISSABLE!
There are 24 treasures in total on the island.

Real Deadeye
Earn an S rank in all games at the shooting range.
Chapters Involved: 3, 9, 11, 14, 15
In-Game Reward: Nothing
MISSABLE!
You can get this achievement in any of the chapters above, but remember that in earlier chapters, not all of the games will be open to you. So, if you want to do all the shooting range games in one sitting, then use the Merchant resting point in Chapters 14 or 15. You do not need to make a perfect S rank to have the achievement unlocked. Just make sure you get to the bonus part of each game, and you get a good amount of targets. There is a video linked below that shows a very fast way for each game at the shooting range:

Jack of All Trades
Complete all requests from the Merchant.
Chapters Involved: 1, 2, 3, 4, 5, 7, 9, 10, 11, 12, 14, 15
In-Game Reward: Nothing
MISSABLE!
There are a total of 19 Merchant requests in the entire main story. If you do all the merchant requests in ONE playthrough, you can unlock this achievement. There is no carry-over of requests to your next playthrough and that is why I am proposing this run to cover them ALL. Remember that you need to do some backtracking for two of the requests. You can use the guide in the following link, where the request notice pick-up location, how, where, when to do the request itself, and how to interact with the merchant to have the request marked as done are shown:

Revolution Wind-Up
Destroy all Clockwork Castellans.
Chapters Involved: All Chapters
In-Game Reward: “The Primal Knife” for the next playthrough.
MISSABLE!
There is a total number of 16 Clockwork Castellan in the entire game and shooting all of them will earn you this achievement. Upon completion, you will have “The Primal Knife” unlocked for your next playthrough and that is what we are looking for in the first run. This is also necessary for the achievement of acquiring all the weapons in the game. Please refer to this video for a thorough guide for all 16:

Remember that you must purchase the item as a "Bonus Weapon" in the "Extra Content Shop" in the game's menu for the next playthrough.
Second Run

In this run, you are going to redo the game in the minimum difficulty, “Assisted”, and get 4 out of 46 achievements along the way. This second run should be a “New Game Plus” run as we need the carryover of your items onto your next run. For this run, now that you are familiar with the game, you need to do the game without using any healing item, without talking to Merchant, and just using your handguns and knives. I know it sounds overwhelming, but with assisted difficulty, it is really not. Since we will be doing this a little faster than the first run, you can complete the game way shorter than the first run and unlock the Sprinter achievement for beating the game under 8 hours.

Before starting the run, make sure to load the last save WITH MERCHANT of your first run to fulfill the requirements before the actual run. You need to upgrade Red9, Blacktail, and Krauser's knife to the fullest. Also, you have to store a sniper with the bioscope in your storage. Finally, you need to fill your inventory with small resources. Once done, finish the current game again, and then start the game as a new game plus. You do NOT need maximum life as I did not have it as well. I dragged myself with half of my full life almost across half of the game. I finished the game with red health status at the end.

Some notes for this run:
1. Upgrade your Armor to the fullest.
2. Gather treasures as much as you can, this will be required for “Gun Fanatic” achievement.
3. Perform headshots as much as you can to save ammo.
4. As soon as you are given boot or kitchen knives, switch your equipped knife with those. You need to keep your primary knife until Krauser fights later in the game.
5. Do NOT worry about resources, be the easiest difficulty of the game and you have all small resources from the last run, it should be more than enough to finish the game.
6. Remember that you can do the merchant's request without talking to him. Just do the requests as much as you can and hold it to the last Merchant. Save your game at the latest merchant and keep going with the game. Once the game is done and you get the achievement for not talking to him, load up your save and this time, talk to him. He will give you all the rewards, hence a better chance for you to get a “Gun Fanatic” achievement.
7. Save A LOT!
8. Even after the run, do NOT sell any of the treasures to Merchant unless you really have to. Once the run is over, you need to combine them for the highest selling price. This is required for getting a “Gun Fanatic” achievement after your third run.
9. Even after the run, do NOT spend your Spinels unless you have to. This is required for getting a “Gun Fanatic” achievement after your third run.
In the following video, you can see a full walkthrough of how this can be done:

Silent Stranger
Complete the main story without talking to the Merchant once.
Please see the notes above for the second run.

Minimalist
Complete the main story using only knives and handguns (Excluding specific battles).
Red9 and Blacktail will destroy all enemies and bosses.
You can use whatever the game throws at you during the run, like canons, harpoons, etc.
Do NOT use any grenades.
Use your secondary knives, like kitchen or boot knives as much as you can to finish the enemies.

Frugalist
Complete the main story without using a recovery item.
With “Assisted“ difficulty and having an upgraded Armor, you are taking less damage. You are not taking any damage if you are in a boat or on a mining cart. I would suggest having 75% of your health up to chapter 10, but that heavily depends on your gameplay style. Remember to save a lot so you can go back and re-do a chapter if you lose too much health.

Sprinter
Complete the main story within 8 hours.
You will get this achievement unlocked by following the guide shared above. If somehow it was missed, do not worry about that. You can unlock this when we do the 3rd run.
Third Run

In this run, you are going to run the game in the highest difficulty, “Professional”, and get 5 out of 46 achievements along the way. This will be your hardest run as you have to start a new game on “Professional” difficulty, not a “New Game Plus”, so no carry-over from your last game.
The objective here is to achieve an S+ rank on professional difficulty, and that is why you need a new game. The achievement will trigger if you complete the game within 5 and a half hours, using only 15 saves. Since difficulty achievements in RE4 Remake are stacked, if you can manage to get an S+ rank on professional, you will unlock ALL difficulty-related achievements. The list is below.

Peerless Agent
Complete the main story on Professional mode.

S+Rank Investigator
Complete the main story on Hardcore mode with an S+ Rank.

Proficient Agent
Complete the main story on Hardcore mode or higher.

Mission Accomplished S+
Complete the main story on Standard mode with an S+ rank.

Promising Agent
Complete the main story on Standard mode or higher.

Of course, you have the option to do multiple runs on Standard and Hardcore difficulty, but with two runs already in your pocket, this should be rather okay for you by now. The time objective is a piece of cake. For the number of saves, remember that you need some strategies for specific parts of the game so make your saves exactly as shown in the video. Remember that you do need to quit the game and load your save from the main menu if you die in the game. Otherwise, your play time will accumulate and eventually you will lose the time objective. I have added a guide for you below to help you through this one:

If you do this run like it was described here, you will get the following in-game rewards:
Deer Antlers
Sunglasses (Round)
Foam Mask
Fight Cap
Glasses (Square)
Iron Helmet
Gas Mask
Knitted Hat
Glasses (Round)
Fight Helmet
Surgical Mask
Armor
Chicken Hat
Chicago Sweeper
Cat Ears
Wolf Tail
Handcannon
Fourth Run

Congratulations on making it this far. Now that you have all the achievements from the main story, we are going to focus on the DLC, Separate Ways. The first run of the DLC is similar to the one in the main story. In this run, you are going to enjoy the story of DLC, Separate Ways, in the minimum difficulty, “Assisted”, and get 3 out of 46 achievements along the way. Needless to say, this would be your longest run, timewise, for the DLC.

Some notes:
1. Remember to save a lot and use all your slots so you can backtrack if something is missed.
2. I strongly advise you to get all the treasures and complete all the requests from the Merchant to have all possible rewards. You need the cache to buy weapons and their upgrades. I have linked separate guides for treasures and Merchant requests in Separate Ways below.

They are relatively easier to get, and Merchant is much more rewarding with Ada in comparison to Leon... not sure why?!
3. Don’t get too easy with your spending. Try to gather as many as resources you can to put together some good money.
4. Important: Keep your save file at the latest Merchant just before the endgame.

Giant Slayer
Defeat El Gigante in Separate Ways.
Chapters Involved: 3
In-Game Reward: Nothing
This is part of the story, so you are going to do it eventually. Just kill the bastard and the achievement is yours.

“It” Kept You Busy
Defeat Pesanta in Separate Ways.
Chapters Involved: 5
In-Game Reward: Nothing
This is part of the story, so you are going to do it eventually. Just kill the bastard and the achievement is yours.

Had Enough of Preachers
Defeat Osmund Saddler in Separate Ways.
Chapters Involved: 7
In-Game Reward: Nothing
This is part of the story, so you are going to do it eventually. Just kill the bastard and the achievement is yours.
Fifth Run

In this run, you are going to run the game in the highest difficulty, “Professional”, and get 4 out of 46 achievements along the way. This run will be a little hard but easier in comparison to the Professional of the main story. You have to start a new game on “Professional” difficulty, not a “New Game Plus”, so no carry-over from your last game.

The objective here is to achieve an S+ rank on professional difficulty and that is why you need to start a new game plus. This will trigger if you complete the game within 2 and a half hours, using only 10 saves. Since difficulty achievements in RE4 Remake are stacked, if you can manage to get an S+ rank on professional, you will unlock ALL difficulty-related achievements. The list is below.

Ada the “S+”py
Complete Separate Ways on Professional mode with an S+ rank.

The Perfect Mission
Complete Separate Ways on Professional mode.

Skilled Agent
Complete Separate Ways on Hardcore mode or higher.

Capable Operative
Complete Separate Ways on Standard mode or higher.

Of course, you have the option to do multiple runs on Standard and Hardcore difficulty, but with 4 runs already in your pocket, this should be rather okay for you by now. For the number of saves, remember that you need some strategies for specific parts of the game so make your saves exactly as shown in the video. Remember that you do need to quit the game and load your save from the main menu, if you die in the game. Otherwise, your playtime will accumulate and eventually, you will lose the time objective. I have added a guide to help you go through these achievements as well:

All done and as shown in the video, you will unlock all the following items by completing this run:

✓ Glasses (Windsor)
✓ Sunglasses (Oversized)
✓ Hair Color (Green)
✓ Bear Hat
✓ Glasses (Round)
✓ Sunglasses (Browline)
✓ Cute Bear Earring
✓ Chicken Hat
✓ Countess Set
✓ Chicago Sweeper
✓ Cat Set
✓ Wolf Set

Extra

Now that you have done all the runs and unlocked all special weapons, it is time to buy them from the in-game credit store for the final achievement. Note that you have to actually buy them and have them in your inventory on a new run. By following the runs mentioned in this guide, you should have the majority of them already in your inventory or had them. All remaining should be bought by Spinels or money directly from the Merchant.

Gun Fanatic
Obtain all weapons.
Once your fifth run is over and you have unlocked all special weapons, load back your last save with the merchant on the second run of the main game and first run of Separate Ways. If you did any requests throughout these runs, have your reward. Then find the best combination of treasures you have gathered to sell them at the highest price. Remember that for this achievement, you JUST NEED TO HAVE EACH WEAPON IN YOUR INVENTORY FOR ONCE. So, if you buy a weapon, sell it, and buy another one, that counts as having both already. That is how you are going to get this achievement. So, if you do not have enough money for the infinite rocket launcher, just simply sell EVERYTHING and buy it.

This is the full list of weapons:

Knives
Combat Knife: Default
Fighting Knife: Defeat Krauser (1st run).
Primal Knife: Destroy 16 Clockwork Castellans (1st run).
Handguns
SG-09R: Default

Thanks 🙏
Yeah, that may be true. For me, the run in Professional to get the S+ rank was the toughest :zagshock:
Some parts of the game got very nerve breaking as I had to repeat them multiple times and resource management was not easy :zagsnooze:

Thank you so much for the guide.
The only thing I may add is: the first run is much funnier if in hardcore mode, because after the first run every run lose the "survival horror" feeling XD

Hi
You have the option to skip the boss fight by running in circles from one room to another. But if you actually killed him and picked up the gem from his dead body, I would say the achievement should have popped up. On my first run, I sprayed nitrogen on him and used rocket launcher to make powder out of him :-)
The achievement was immediately there for me.
