Before you begin.
This game is glitchy. It's a good VR game, especially if you're into VtM, but the fact of the matter is the game is glitchy and those glitches *can* derail your quest for getting all of the achievements. The two achievements that I came across that became glitched were Apex Predator and Not a Nice Person. I will provide details in the section with those specific achievements.

I recommend backing up your save files periodically in case you encounter a progress-breaking bug or glitch. The default location of the save files is: C:\Users\[name]\AppData\Local\Lollipop\Saved\SaveGames

Very minor story spoilers ahead.
Story Achievements
You should unlock these achievements by progressing through and finishing the story.


Sewer Rat
Meet Pietro.


The boss is dead
Send Elena to her final death in Payback.


Into the sunset
Finish the game's main storyline.
Gameplay Achievements
These achievements are all related to gameplay mechanics. Most of them you will likely get without trying.


Rapid Reflexes
Blink while falling. Exactly what it says. Blink when falling off a ledge.


Bonk!
Hit an enemy in the head with a throwable object. Throwables are lying all over the place; pick something up, get close to someone, and throw it at their head.


Alas, poor Yorick
Pick up a skull. There are skulls lying around in various places; just pick one up.


Learning to Swim
Die by falling into water three times. Exactly what it says.


Death from above
Blink attack from above five times. You will likely do this naturally as you deal with enemies. You just have to be above them when you blink attack them.

Oddly Specific Achievements

Hands where I can see them
Finish Server-Side Issues without using the crossbow. Do exactly what it says. This achievement unlocked for me without my really trying to do it so it's possible it may be glitched in your favor.


Have your cake and eat it
Free the prisoner in Server-Side Issues, then drink their blood. If you go into the basement of the fundaco there is a prisoner being held in a cell. A switch on the wall will open the cell and you can feed on the prisoner. They have way more blood than a normal mortal.


Brain Damage
Hit Pietro in the head in eight different conversations. I suggest working on this from the beginning of your playthrough because there are only so many conversations with Pietro and you cannot go back and replay the sections that happen between individual missions. If you don't get this on your first try you'll have to start a new run. I would just pick up any random object before getting close enough to Pietro for him to start talking but you can always walk away from him and he won't progress. Hit him in the head with something. I do not know if your fists count. If you do this every time you talk with Pietro you will get the achievement before the end of the game.


Rat Bowling
Kill three rats by throwing bottles at them. There are plenty of bottles and plenty of rats; just keep your eyes open for opportunities.


Oblivion takes you
Use Shadow Trap three times in The Anziano. Exactly what it says.


I am thirsty
Finish Work for Hire without drinking any blood. I recommend trying this after you've acquired some upgrades and saving any bolts or abilities for taking out the target at the end. You cannot drink any blood, not even from rats.


Barfighter
Hit an enemy with a bottle, at The Attendente's Bar. Do exactly what it says, however you MUST actually be inside the bar for this to work. The description is somewhat vague as The Attendente's Bar is the name of the whole level. You must be in the building where you hide the phone.


Tomb Raider
Open the tomb in the Closed-down Elysium. At some point during the story you will see a new red icon in Pietro's Hideout and there will also be a note for you about a closed-down Hecata Elysium. You need to finish this side quest to unlock all of your disciplines anyway so if you're going for a true 100% you will be doing this already. The levels are primarily puzzles but they are not very difficult. There are videos online showing people playing through them so I won't go into details here.
Collectible Achievements
All of these achievements are for collecting all of a given category of collectible. All collectibles except for Hecata Medallions emit a tone when you are near them. You can turn the music down or off to help you with this. Some of the collectibles require a minor puzzle or intimidating NPCs (more on this later) but generally you will find them all if you explore every part of a level and listen for the audio cue. The Hecata Medallions are a little trickier but they will always be hanging from something so the vast majority of them are above your eye level with few exceptions. I believe there is at least one video online showing the locations of collectibles so I will not go into detail about locations. Just be aware of what you're looking for. When you get to the point in the story where you can replay missions you can actually see how many collectibles and medallions are in the level and that helps immensely with collecting them all.

A note about the Hecata Medallions: Based on Steam achievement percentages it doesn't seem like you need to shoot all of the medallions to get Apex Predator, but you do need to shoot them all to get the relevant achievement so you may as well do it anyway if you're going for 100% achievements.


The Hecata
Find all of the Hecata masks and listen to the story.


Pietro
Find all of Pietro's paintings and listen to his story.


Justice
Find all of Justice's ornaments and listen to their story.


The Banu Haqim
Find all of the Banu Haqim symbols and listen to the story.


Potent Blood
Fully upgrade your health.


Elena
Find all of Elena's diaries and listen to her story.


Tommaso
Find all of Tommaso's goblets and listen to his story.


Marksman
Shoot all the Hecata Medallions in Venice.
Challenge Achievements
These achievements are granted for completing all the relevant challenges in the game. You will be able to see your optional objectives/challenges for a level when you open menus. You can complete these while replaying missions so it's okay to wait until you have more disciplines before trying to tackle them. You must complete all challenges in order to get 100% for Apex Predator. You also need the experience points from them to unlock all disciplines which you also have to do for Apex Predator.


I don't believe in violence
Clear all Pacifist objectives in the game. You cannot harm mortals in a level in order to pass these. You can use the sleep bolts on people but you cannot kill or drink from them.


I am invisible
Clear all Predator objectives in the game. You can kill as many mortals as you want or need to, just don't get spotted. You'll know if you were spotted because the objective will show as failed. My strategy for this was to simply kill everyone before they had a chance to see me if there was no clean way to sneak past them. Security cameras can easily ruin an attempt so I recommend using the upgraded corrosive bolt on them if necessary.


I am untouchable
Clear all Untouchable objectives in the game. To complete these you cannot be hit at all during the level, this includes boss fights. This isn't as hard as it sounds once you know how to defeat each boss. If you get hit during a boss fight just pause and reload last checkpoint instead of doing the level all over again.
Not a Nice Person
My least favorite achievement and the one that gave me the most trouble.


Not a Nice Person
Successfully intimidate every possible NPC.

Sounds pretty easy, right? Hopefully it is for you but this achievement is sketchy with how it tracks and grants. There are six NPCs in the game that you can intimidate. In order to use the Intimidate dialog option you have to have at least one level in the Intimidate discipline. If you are going for this in your first playthrough I recommend getting Intimidate to level three as soon as possible.

The NPCs that you can intimidate are Lorenzo in The Phone Call, the guard at the gate in The Attendente's Bar, the Hecata traitor in Server-Side Issues, the guard at the greenhouse door in Meet the Boss, the guard behind the locked door in Work for Hire, and the Hecata traitor again in Shadows and Whispers. You must intimidate all of them except for the traitor in Server-Side Issues and the greenhouse guard in Meet the Boss if you want to get all of the collectibles. You should still seek to intimidate everyone for the sake of this particular achievement.

It took me three playthroughs to get this to unlock and in order to do it I had to successfully intimidate each NPC as I came across them the first time. Replaying levels does not seem to work for this. You can successfully intimidate Lorenzo with only one level of Intimidate but there seems to be a level of randomness to it because I have also failed the attempt with only one level. If you fail to intimidate Lorenzo on the first try you may want to restart the level or at least go to the last checkpoint. The achievement unlocked for me after successfully intimidating the NPC in Work for Hire. I do not know if this was yet another glitch but you may have to intimidate the last NPC in Shadows and Whispers or it could unlock after five NPCs; your mileage may vary.

This one can be frustrating if it fails to unlock but if it doesn't work on your first playthrough just continue toward Apex Predator because it is a lot easier to speed run the story just to intimidate NPCs than it is to collect all the collectibles, unlock all the disciplines, and do all the challenges again.
Apex Predator

Apex Predator
Finish the game with 100% completion.

In order for the game to count 100% you must complete the story, collect all collectibles, unlock all disciplines, and complete all challenges. You can see the progress on your save file within the game menu.

I had a major glitch on my first playthrough that made it impossible for me to complete this. At some point I stopped accumulating experience points which made it impossible for me to unlock all of the disciplines. I recommend backing up your saves frequently and keeping an eye on your experience to make sure it is adding up correctly.

A few notes: You cannot unlock all of the disciplines before defeating the final boss. There simply isn't enough experience in the game to do this prior to defeating them, even if you collect every collectible and do every challenge. When I finally unlocked Apex Predator I had to defeat the boss a second time for it to register as me "beating the game *with* 100%". If your save file says 100% and the achievement hasn't unlocked, try beating the boss again. They really aren't that difficult unless you're trying for the Untouchable challenge.

Seems like a pretty good guide. I started the game today. Not sure if I'll attempt the 100% because of some time consuming achievement that need the whole run and are not dependent on chapter select. I did 100% Wraith: the Oblivion - Afterlife which is from the same developers but I think this game is more complicated to 100%.
