This guide will help you beat the game and earn all of the achievements. Includes info on vermin spawn locations in levels 1-4.

Vermin Locations (Walkthrough)
The basic gameplay of Perfect Vermin involves destroying "vermin" in an office setting. There are some variations on this throughout the five levels of the game. Below I have listed all the possible spawn locations of vermin that I am aware of.

If you are being told to hunt 10 or 11 vermin in any level besides level 4, or if you are told to return to the elevator seconds after exiting it the first time, then your system does not meet the base requirements for Perfect Vermin, which will prevent you from earning all of the achievements. Unfortunately, these issues stem from the programming of the Unreal engine itself, so there is no way to make it run properly on a computer that does not meet the minimum specs. You should still be able to complete the game, but the experience will not be as good.

Level 1a and 1b - 5 Vermin each
Office #1. These will both be the same, since you do the same level twice.

-Chair left of the couches
-Chair outside break room
-Chair in break room
-Refrigerator in break room
-Chair in room across from elevator room
-3 different chairs in cubicle room on the right half if walking from the break room
-Table in room right of the bathroom
-Toilet in third stall in bathroom (indicator will be red not green)

Level 2 - 6 Vermin
Office #2.

-Chair just outside elevator
-Chair just outside bathroom
-Chair in bathroom
-Printer in room right of the bathroom
-Door in room by the windows
-Desk by the windows
-Chair by the corner desk by the windows
-Chair outside the bathroom
-Chair in between the cubicles
-Toilet in the break room

Level 3 - 5 Vermin
Upside-down office #1. The vermin will be the same 5 you destroyed in level 1.

Level 4 - 10 Vermin
Office #1 and #2 with a split screen. The vermin locations are the same every time.

Right View:
-Chair by bathroom
-Chair in bathroom
-Door in room by the windows
-Desk by the windows
-Toilet in the break room

Left View:
-Chair outside break room
-Refrigerator in break room
-2 chairs in cubicle room
-Toilet in third bathroom stall

Level 5 - 20 Vermin
There is no point in destroying the vermin in level 5, so I won't list them here. Try to get into the secret door (see "Unborn" below) or wait for the timer to run out.

Level "6"
Destroy the white things.
Achievements
You will have to play through the game several times to earn all of the achievements, but they are overall not too hard if you know how. I have put these in the order I suggest trying to earn them. They all trigger immediately unless otherwise noted.

As of the posting of this guide, Perfect Vermin has limited profile features, meaning the following achievements will not show up on any profile showcases, banners, or global achievement stats until Perfect Vermin stops having limited profile features, which may never happen since it's a free game.

Achievements cannot be unlocked in the Terminal Velocity beta version of the game.

First Playthrough
All of these can be achieved or at least reasonably attempted in your first playthrough.


Just Stop
Find a metal fire door and hit it three times. The fire door is near the couches in level 1.


Breakdown
It's a lot of fun to just smash things in your first playthrough anyway, so be sure to smash 200 objects in the same run. Don't worry too much about vermin or the timer until you do.


Lights Out
This one is entirely possible on the first playthrough. Find all of the light switches in each level (1-4, sometimes triggers after level 3) and break them with your sledgehammer; there is a unique sound effect. Every room has at least one, and there is usually one by the elevator (Level 1/3 have 9, Level 2 has 6, Level 4 has 15). Make sure to break all the switches at the same time you destroy all the vermin in each level. Progress is not reset if you have to retry a level. If you keep running out of time, just turn off timers from the settings menu.


You Are Likely Dense
At the end of level 1b, after you are told to enter the elevator, do not enter it and wait. The speaker will ask you if you are dense, earning you this achievement.


Unborn
This requires you to enter the secret door in level 5. This door is essentially across from where the door to the elevator room is in level 1 (seen below). Sometimes standing on the very edge of the break room doorway and then holding W will get you there. The secret door will ONLY be there the first time you enter this room in each run. You will probably have to do this in a subsequent run, but it's good to know about it early.

In Loving Memory
This is earned after beating the game once, and it will trigger during the credits.


Second Playthrough and Beyond
All of these are better to try for after you've beaten the game at least once. Don't forget to grab any you missed from above as well.


Newsworthy
Complete levels 1-4 without letting the timer run out (timers must be on). This is fairly easy if you follow the guide above about where vermin spawn; the hardest is level 4. For level 4, I suggest getting all 5 vermin on the right view then all 5 vermin in the left view. This achievement will trigger at the end of the credits if you earned it.


Surgical Precision
Destroy only vermin and doors in levels 1-4 (sometimes triggers after level 3). If you follow the guide about vermin spawns, this should be straightforward, as most vermin aren't too close to other breakable objects. The trickiest one is level 4, where you have to be sure you won't break something in the opposite view as you destroy a vermin in one view. Turning timers off can make this one easier.


The Story of a Lifetime
Beat the game within 4 minutes and 30 seconds (timers must be on). Arguably the hardest achievement (if it doesn't trigger preemptively). Get through each room as fast as possible, remembering the spawn locations in level 1 for level 3. You will probably have to make it into the secret room in level 5 to be fast enough. You can look up a speedrun of this game to get an idea. This achievement will trigger when you enter the doctor's office.


Perfect Vermin
This one should trigger after you destroy 1-2 vermin in a fresh run after having gotten the other 9 achievements. Sometimes a restart of the game is required as well, or pressing the Steam overlay (Shift + Tab) while in game. There are discussion threads with more info if this one is not triggering for you.


Conclusion

It won't unlock Perfect Vermin, restarted, started many new runs destroying 1-2 vermin, opened the steam overlay, nothing. still at 9/10

It's more than possible to run the game on a computer that doesn't meet the minimum required specs, but it doesn't run correctly. That happened to me at first.

I'm sorry to hear that, and I'm not sure why that would be. The only thing I can suggest is checking the integrity of the game files and/or uninstalling and reinstalling the game.

For some reason story of a lifetime isnt triggering- timed the last run myself. I have all the other achievements, and this is bugging me lol
