"As vehicles age they produce more emissions. To keep an old vehicle running with lower emissions. Changing Maintenance to 'Very High' will reduce emissions to the original value for the vehicle.

You can see the effect by comparing these two 21 year old buses. The Maintenance level was only changed a year or so before this check."

Another thing is that I think the 5% redduction in emissions for an extra 50% maintenance cost is tiny.

I'd agree that the value of the vehicle should be higher if well-maintained, but it seems like it's not factored in at the moment.
Also, emissions make cities grow less quickly - if you look at any city's detail there's a negative growth score by emissions.

How do cities react on emissions? Are residential areas with high emissions just not growing or are they converted into industrial zones that don't care about noise?
