Initially though, he is actually Bill from the info_l4d1_survivor_spawn entity.
In order to allow the game to spawn him in if other players are using Bill, all survivors using the Bill slot are momentarily changed to an empty survivor slot before spawning.
After that, he's set to the empty slot, and given his custom model and name.
Map transitions don't handle 5th or higher survivor counts and will not spawn the excess in the next map.
The script has to spawn him in manually again in the new map as well as storing his inventory info to give it back to him.
This wasn't initially implemented until the current CF update where Mike actually tags along as a 5th survivor if Cola is brought to the saferoom.

Technically Mike's only a Bill replacement in the model he uses which is based off Bill's model and his voice set which was manually given to him via talker context.
He uses an empty survivor character slot that allows his name to be shown, the downsides being that he will not have working arm models or HUD icons due to them being client-side and impossible to control, and is unable to be defibbed, forcing the script to always ragdoll him on death. (Asking JAiZ for lore reason he decided to hand-wave it as Mike actually not being a carrier)
Only Bill's voice files were truly replaced in the files of Cold Front, Bill's model and icons remain intact.
