Refresh rate fix running by itself does not appear to work correctly on newer (P-core, E-core) systems. On my 12900k and 4090 setup the framerate will bounce around between 44fps and 82fps with just Refresh Rate Fix, leading to a stuttery experience.
SekiroFPSUnlockandMore tool running by itself will not increase Refresh/FPS above 60hz in fullscreen mode (but the other options such as FOV increase etc will work, you just have to ignore the version error that pops first time it runs).
Whereas using both mods/tools together gives a flawless 120hz/120fps experience at 4k max settings with zero framerate drops/stutters (using an LG C2 as display - note there appears to be plenty of headroom to spare so if youre running a higher refresh rate monitor with similar cpu+gpu combo then should achieve max fps on that as well).
I've heard similar magic for Elden Ring(with all the risks since FS/Bandai Namco are a bit anti-player and ban you from going online if you modify literally anything at all, thanks to who they are pandering to/how the netcode and server are configured), however, I guess I'm wondering if you can slap DXVK in here for compatibility if FSE methods are causing hang-ups.
In the case of both Nioh 2 and FNV, I found better overall advantages and performance in borderless windowed. I use a free variant of "Windowed Borderless Gaming" that works fine. However, I think AMD GPUs don't work with DXVK in Windowed mode, due to no supported flip method present in the drivers. With a bit of careful tuning in the GPU control panel, and with DXVK, you can get FSE-like advantages with greater performance and stability, i.e. alt tabbing is less of a potential mess.
Just tossing in my two cents, and I love that Sekiro is getting mod love/attention. I don't love that patches come out to intentionally break mods; it's an increasingly common way to censor community creations and mods, to maintain exclusive control of the public's perception of the product.
Side note, potentially related: Valve recently made a move to change how publishers/devs populate the store page with advertising material like trailers and screenshots, particularly. Patches like this could easily be a knee-jerk side-effect of some heated corporate meeting at some office in Japan far away, in which the idea that they're "losing control" must have necessitated some subtle changes to how they conduct business.
This method I mentioned becoming increasingly common would have been something they've known how to do, and have done in a more subtle fashion within the corporate sphere. This however quickly detaches the player-base from its community, in hopes of weakening the appeal of the storefront/Steam client.
Totally on a level of conspiracy theory to most who will read through this, I realize, but is not unlike how the world of enterprise is conducted. Just remember, the boss rush mod and costume mods existed before it was part of an official patch, taking credit and attention away from those who originally implemented and advertised the idea to the playerbase. They stole credit, and it is often justified by saying "well, it's their game" and this is correct. It is not, however, being nice to us, and they could do better than this.
We deserve better than needlessly antagonistic disruption of the modding communities, and this further motivates people to play their games fully disconnected from the internet. Me, I'm going offline now. I don't sub to anything I comment on anymore, not since Elden Ring came out lol. Best of luck all. Let's hope this is the last update.

I always had no problem getting it to work, which is why I burst out laughing when it read "PSA".
First enter the game and make sure you put Screen Mode to 'Windowed'.
Finally, Alt-Tab to the Utility, and click on 'Borderless window'. Ensure 'Fullscreen stretch' is grayed out.
Click 'Patch game' as needed.
Bit of a necro thread but this method mentioned at top of thread worked for myself and several others as described. It also applies to getting it working in the games native "Fullscreen exclusive" mode and not windowed or borderless windowed mode you mentioned (I could get 120fps to engage in both those without using the workaround mentioned but native HDR wouldnt work in win10 in borderless or windowed mode with Sekrio, so workaround was only way to get 120fps, exclusive fullscreen AND HDR working for me on win10+LG C2 TV combo).
