I may be wrong, but this is how I understood it (Btw, if you choose to "ignore" the Stranger it asks your name, you could probably find out more about the backstory... I'll leave that up to you to discover) : Your main character in this game is depressed, with Despair being her "doctor" giving her dark advices that would help her escape the place she is in (which could be symbolism for the world in real life) with the use of a noose. Also, the whole "Stranger" character is also "Despair" on the other side of the wall being unhelpful as always with the suggestion of killing yourself in order to escape the word. I think what the game is trying to portray is depression and mental illnesses (like how Despair asked if you were worried about the voices inside your head). It's actually pretty creepy to think that Despair is the only other humanoid character present in the game, and although the plot is already as dark as it is, Despair gave you a fallacious choice of staying in that awful place forever, or using the noose to exit. And of course, you chose the latter to proceed with the ending which basically is just reverting you back to the main menu. Also, I think the beetles represent your "will" to get better, as the gameplay of the beetle comprises of looking for keys and exits, and also shooting the creatures called "hopelessness". In the end though, the beetle turns into a gravestone as you choose to use the noose. I guess this is why it's called "Life Beetle". It is also worth mentioning that "Despair" said that he and the beetle are the only ones with you in that place; but your character chooses to let Despair get to their head in the end. ): Well, hope you understood all that, haha.
