Steam Leaderboards have been bugged for a really, really long time so it's not really accurate at all. I believe most of the Global Scores are from cheats, some are from glitches, lag, exploits, Race Mode corrupt save files etc. With all those in mind it probably caused the score to be really inaccurate.
As for what the score is or what is means, i'm unsure since it's not clear at all though Super Meat Boy:Forever has the leaderboard score return and the score is calculated based on time, rank, and pacifers (bandages) collected.
In Forever, If you get an A+ Rank that's 25000 score if you get S+ Rank (No deaths) you get 100000. Then Pacifiers + Time gives you a little bonus alongside that.
I'm taking a guess but that similar system is how it works for Super Meat Boy but due to the broken leaderboard it messed up the tracking system.
