Last unlockable note?

I am missing the last note, the one right under Dr Warren in the list. I allready got all endings and have played through hunting for it more than once. Can anyone tell me where it is?

For anyone interested, the achievement just "dropped" when i was taken by shadows in the red plane. I think it was one of the three notes in dr warrens house and it had not registered, for some reason.
