i already removed all of my modes but when i am trying to open game it just crashing how can i remove problem

I finished a game and closed it, then I realized that ALL my Steam achievements had disappeared. I tried opening and closing the game, starting and finishing a run... And nothing, I still have the progress within the game, but the Steam achievements are gone. Does anyone know how to fix this?...

I had The Dead Sea Scrolls as my spacebar item. I was doing great, using the DSS every room to get all these benefits and suddenly, it teleports me out of the room. I can't get back, and it won't be marked as done. ...
