Keep accidentally killing someone in Knife Of Dunwall Whale Level
It always says I killed one person despite using non-lethal methods. I feel like they glitching out or something and it's preventing me from getting the Cleaner Hands achievement.

maybe when you throw an unconcious body too far it dies?

Happened to me, get the stats mod because that tells you exactly the moment the person died, for me it was in the whale house area and i remember it happened when i ran to a certain room which probably despawned stuff causing a body to fall under the map, i left the area and once the outside loaded then it was fine

That's where it might be happening, I sometimes see the larger guys glitch out a little after rag-dolling but didn't think that much of it at first.
To prevent unconscious enemies from dying, you can also avoid knockouts as much as possible, it's easier than it sounds too.
You have to be cautious. Enemies that you put out of commission you must place in positions where they will no fall victim to rats. Remember that when you incapacitate enemies via non-lethal means may die as a result of your actions if you leave them where they fall, and plan accordingly. There is also the annoying yet unavoidable possibility that they may die unless you take steps to ensure that doesn't happen.
