@OP - Right click on the game in your Steam Library, Manage -> Browse Local Files

Create an empty text file named imma
Right click on this new text file and click Properties

And yes it works for me. This is all I had to do. If you really want I'll hop in-game and show a screenshot of it working - there's different text if you successfully mate parents who are siblings.

Works on my machine.
Open the game folder, open cmd in it (you can type cmd in explorer's address bar and hit enter), and run to just make the file without having to deal with gui nonsense.

You have to enable file extensions. Click View -> Show -> File Name Extensions.

You should then be able to rename the file with the .degenerate file type.
